SendSlim Clinic is a medical weight-loss practice that provides care only in states where our clinicians are licensed.

Right now, SendSlim serves adult patients in:

  • California
  • Nevada

If you are not physically in one of these states at the time of your visit, we cannot provide medical care to you through SendSlim.

1. Where We Currently Provide Care

California

SendSlim provides telehealth medical weight-loss services to adult patients who are physically located in California at the time of the visit.

Our clinicians who treat California patients are licensed by the appropriate California licensing boards, such as:

  • California Board of Registered Nursing (for nurse practitioners)
  • Medical Board of California (for physicians), as applicable

Our practice mailing location is in Modesto, California.

Nevada

SendSlim also provides telehealth medical weight-loss services to adult patients who are physically located in Nevada at the time of the visit.

Our clinicians who treat Nevada patients are licensed by the appropriate Nevada licensing boards, such as:

  • Nevada State Board of Nursing (for nurse practitioners)
  • Nevada State Board of Medical Examiners (for physicians), as applicable

2. Telehealth and Your Location

Telehealth laws require that:

  • You must be physically in a state where your clinician is licensed at the time of your visit.
  • We may ask you to confirm your location at the start of each telehealth appointment.

If you are traveling or temporarily staying outside of California or Nevada, we may not be able to see you by telehealth until you are back in one of these states.

3. If You Live or Travel Outside California or Nevada

If you live outside California or Nevada, or are traveling outside these states:

  • You are welcome to use this website for general information and education only.
  • We cannot provide telehealth medical care or prescription services to you through SendSlim while you are outside California or Nevada.
  • You should seek care from licensed healthcare professionals in the state where you are located.

We may add more states in the future. Any updates will be posted on this page.

4. Types of Clinicians at SendSlim

SendSlim Clinic uses licensed healthcare professionals such as:

  • Nurse Practitioners (NPs)
  • Physicians (MD/DO)
  • Other licensed team members as appropriate under state law

All clinicians are:

  • Licensed in the state(s) where they provide care
  • Practicing within the scope of their license and training
  • Providing services under the policies and clinical standards of Affection Health Care LLC / SendSlim Clinic

5. How to Verify a License

Most state licensing boards allow the public to look up a license online by name or license number.

If you want to verify a clinician’s license:

  • Visit the licensing board website for California or Nevada
  • Use the board’s “license lookup” or “verify license” tool
  • Search by the clinician’s name or number (if you have it)

If you are a current or prospective patient and need help finding the correct board or verifying a license, you can contact us and we will provide you with the basic details you need to perform your own search.

6. Questions About States of Practice

If you are unsure whether we can see you:

  • Check where you will be physically located at the time of your telehealth visit.
  • If you will be in California or Nevada, we may be able to provide care.
  • If you will be outside these states, we cannot provide telehealth medical care through SendSlim during that time.
