1. What is SendSlim Clinic?
SendSlim Clinic is an online medical weight-loss clinic. We see adults by video or phone and help them lose weight in a safe, medical way. We use lab tests, real treatment plans, and sometimes weight-loss medicines when they are safe and appropriate.

2. Where do you see patients?
Right now, we can treat patients who are physically in: California Nevada Even though visits are online, you must be in one of these states at the time of your visit for us to provide care.

3. Who runs SendSlim?
SendSlim Clinic is operated by Affection Health Care LLC, the same company behind: DOT Modesto Unity Pain Management SendClinic (telehealth urgent care) Care at SendSlim is led by a board-certified Nurse Practitioner with a supervising physician, and other licensed clinicians as needed.

4. Is SendSlim for emergencies?
No. SendSlim is not an emergency service. For chest pain, trouble breathing, stroke symptoms, severe allergic reactions, thoughts of self-harm, or any life-threatening problem: Call 911 or go to the nearest emergency room right away. Do not wait for a telehealth visit or a portal message.

SendSlim may be a good fit if you: Are 18 years or older Live in California or Nevada Struggle with overweight or obesity Want medical help with weight, not just another diet app Are open to lifestyle changes and, if safe, possibly medication

6. What health problems do you focus on?
Many patients we see have: Pre-diabetes or diabetes High blood pressure High cholesterol Joint pain, back pain, or low energy made worse by extra weight Fatty liver or other weight-related issues We do not treat every kind of medical problem. If a different type of specialist is needed, we may recommend you see them.

7. Can you help me if I have tried many diets before?
Yes. Many of our patients have tried many diets in the past. Our goal is to understand your history and help you find a safer, more steady plan that fits your real life. We do not promise quick, dramatic weight loss. We focus on safe, steady progress and long-term health.
How It Works

8. How do I get started?
To start: Go to our Start Your SendSlim Program page. Fill out the online intake form (your history and goals). Choose a date and time for your first telehealth visit. You do not need to call first to start.

9. What happens in the first visit?
At your new patient visit, your clinician will: Review your intake forms and health history Ask questions about your symptoms, lifestyle, and past diets Check for risks and other health problems Decide if labs are needed Talk with you about a plan, including lifestyle changes Discuss whether weight-loss medication might be safe for you This visit is usually 45–60 minutes by video or phone.

10. What happens in follow-up visits?
In follow-ups, we: Review your weight and symptom changes Look at lab results (if done) Check for side effects if you are on medication Adjust your doses, timing, or plan as needed Encourage you and help you stay on track Follow-ups are usually 20–30 minutes by video or phone.

11. Do you prescribe weight-loss medications?
Sometimes. We may prescribe weight-loss medicines if: We believe they are safe for you You meet medical guidelines for that drug There are no strong reasons not to use it Not every patient will get a prescription. No specific drug (including GLP-1 medicines like Wegovy or similar) is guaranteed.

12. Will I definitely get a medication on my first visit?
No. We will never promise a medicine before we review your: Health history Medications Lab results (if needed) Risks and benefits Sometimes the first step is labs and lifestyle changes, not medicine. Sometimes we decide a medicine is not safe or not needed at all.

13. What labs do you check?
Labs are different for each person, but common tests may include: Blood sugar (like A1c or fasting glucose) Kidney and liver function Cholesterol and triglycerides Thyroid tests Other tests as needed for your health We may also review recent labs from your primary care provider if they are up to date. Labs are usually billed by the lab company, not by SendSlim.

14. How much do medications and labs cost?
Costs can vary a lot based on: Your insurance plan The lab or pharmacy you use The specific tests or medicines We do not control lab or pharmacy pricing. We will try to explain likely costs so you can make informed choices.

15. How much does it cost to see SendSlim?
Current standard prices: New Patient Weight-Loss Visit: $179 Follow-Up Visit: $119 We also offer: SendSlim Starter Bundle (3 visits – 1 new + 2 follow-ups): $379 Ongoing Care Plan: $129 per month Includes 1 follow-up visit per month + messaging for simple, non-urgent questions Medication, labs, and devices are not included in these fees. 16. Do you take insurance?
SendSlim is mainly a cash-pay telehealth clinic. We do not bill your insurance directly for visits. On request, we may give you a receipt or superbill you can send to your plan. We cannot promise that your insurance will pay or reimburse any amount. Any coverage or reimbursement is decided by your insurance company, not by SendSlim.

17. Can I use my HSA or FSA?
In many cases, you can use: HSA (Health Savings Account) cards FSA (Flexible Spending Account) cards for eligible medical expenses. Check with your plan for the most accurate information. We accept major credit/debit cards and many HSA/FSA cards as payment.

18. How do I join my telehealth visit?
Before your visit, you will get: An email and/or text with a link to your visit (for video), or Instructions for a phone call if doing audio-only You will need: A smartphone, tablet, or computer Internet or phone signal A private, quiet space if possible Click the visit link a few minutes early to test your camera and sound.

19. What if my video or audio does not work?
If your video stops working: Try closing other apps Check your Wi-Fi or data Rejoin the visit using the link If it still fails, we may: Switch to a phone visit, if appropriate, or Reschedule if we cannot safely finish the visit If the problem is on our side, we will work with you to fix it or adjust fees if needed.

20. Can you promise how much weight I will lose?
No. We do not promise: A certain number of pounds A certain clothing size A certain time frame Every person is different. Our goal is to help you: Lose weight safely Improve your health risks Find a plan you can live with long-term

21. Will you work with my primary care doctor?
Yes, if you agree. With your permission, we can: Share records or summaries Send lab results Coordinate your weight-loss care with your primary provider We encourage all patients to keep a regular primary care provider for vaccines, screenings, and other health needs we do not cover.

22. Can I stop using SendSlim whenever I want?
Yes. You can stop booking visits or cancel your ongoing plan as allowed by our Refund & Cancellation Policy. Keep in mind: Stopping visits may mean we cannot safely refill medications. It is best to talk with your clinician before you stop any medicine or plan.

23. What if I have a complaint or concern?
